栄養障害後脳症として発症したWernicke脳症の1例

A Case of Wernicke's Encephalopathy due to Malnutrition

　はじめに

　Wernicke脳症において上眼瞼向き眼振が認められることはよく知られている^1）。今回われわれはダイエットによる栄養障害に伴い，上眼瞼向き眼振および失調性歩行を示したWernicke脳症の1症例を経験したので，得られた神経学的所見，特に眼球運動を中心に報告する。

We describe a case of 32 years old female with Wernicke's encephalopathy due to thiamine defi-ciency from malnutrition, and presented with dizzi-ness and upbeat nystagmus.

Neuro-otological examination indicated lesions of the cerebellum and brainstem. Although the level of thiamine was normal on thiamine test, we diagnosed this case as a Wernicke's encephalopathy due to disturbance in the activity of thiamine.

The patient was treated with thiamine and bac-lofen and her dizziness had almost completely disappeared. However, the upbeat nystagmus during upward and downward gaze persisted ever after treatment.
