喀痰細胞診を契機に発見されたTis上顎洞扁平上皮癌の1例

A Case Report of Tis Cancer of The Maxillary Sinus Diagnosed by Positive Sputum Cytology

　はじめに

　喀痰細胞診は早期に肺癌を発見することを目的として，胸部X線写真撮影と同時に肺癌検診の一部として広く行われている。喀痰細胞診で陽性とされ精査を受けても，場合により肺には癌が発見されないことがあり，耳鼻咽喉科領域の精査を行うことにより病変部が確定されることも稀ではない。今回われわれは，喀痰細胞診でclass Vの結果を受け，他院で肺および頭頸部領域の精査を受けたが病変部を確定できず，当科で上顎洞試験開洞術を行うことによって初めて扁平上皮癌の診断を確定することができた上皮内癌の1例を経験した。

Sputum cytology is widely used to find the malig-nant lesion in upper and lower respiratory tract.In lung cancer mass screening, it was used with chest X-ray examinations to discover lung cancer of early stages. In this report, a 62-year-old male showed positive sputum cytology and the lung and head and neck region were examined. However, the malignant lesion was not detected at the previous examinations. Squamous cell carcinoma in situ was finally detected by exploratory maxillotomy.
