下顎骨中心性扁平上皮癌の1例

Primary Intraosseous Squamous Cell Carcinoma of the Mandibular Bone:A Case Report

　はじめに

　顎・口腔領域にみられる悪性腫瘍はそのほとんどが粘膜上皮由来の癌であるが，ごく稀には顎骨内から発生することがあり，顎骨中心性癌と呼ばれている^1）。今回われわれは下顎骨原発の扁平上皮癌で，その切除範囲設定に難渋した症例を経験したので，その概要に若干の文献的考察を加えて報告する。

A 76-year-old male presented with a mass and numbness in the submandibular region. Pantomo-graphy and CT scan revealed a radiolucent area in the left mandibular body although there was no apparent mucosal legion in the oral cavity or naso-pharynx. No primary lesion was found in spite of intensive general examinations. FNA sample obtained from the mandibular bone was suspected as squamous cell carcinoma. Mandibular resection and neck dissection were performed under diagnosis of intraosseous squamous cell carcinoma in the mandible. It was so difficult to determine the surgi-cal margin preoperatively that additional man-dibular resection was required.
