耳下腺内顔面神経鞘腫の1例

Intraparotid Facial Nerve Schwannoma:Report of a Case

　はじめに

　耳下腺内顔面神経鞘腫は頻度の少ない疾患である^1〜4）。今回，顔面神経本幹由来の神経鞘腫を経験したが，治療方針としては神経を腫瘍とともに温存し，今後の経過により再手術などを検討することとした。文献的にも術前に診断することは困難で，すべての報告例で術中に厳しい判断をせまられ，1回の手術で完全摘出を試みた例と部分切除で病理を確認し経過観察とした例が報告されてきた^1〜14）。今回われわれは術前診断に関する工夫と治療法について検討したので報告する。

A 59-year-old male presented with an asymptomatic mass in the parotid region found by MRT which was taken for examination of hisvertigo. Surgical excision was performed, and the tumor was schwannoma originating from the main trunk of the facial nerve continuing into the stylomastoid foramen. We intended to preserve the tumor around the facial trunk, but a partial resec-tion for pathological study was performed.

Preoperative diagnosis using axial and coronal MRI, echography, and fine needle aspiration biopsy should be carefully performed for such neurogenic tumor.
