気管偏位で発見された甲状腺乳頭癌を合併した頸部胸腺腫の1症例

Cervical Thymoma with Thyroid Papillary Adenocarcinoma:Report of A Case

　はじめに

　甲状腺癌を合併した胸腺腫の報告は必ずしも稀ではないが，胸腺腫が頸部に存在する例については報告例は少ない。今回われわれは気管偏位を伴った頸部胸腺腫と甲状腺乳頭癌の合併例を治療する機会を得た。診断は，初め甲状腺腫瘍の上縦隔進展と考えたが，CTおよび^99mTcと²⁰¹Tlによるシンチグラムを検討した結果，2つの腫瘍から構成されている可能性が考えられ，頸部からの摘出術により一塊として摘出できた症例を経験したので報告する。

Cervical thymoma with thyroid papillary adenocarcinoma is a very rare condition. We herein reported the nineteenth case of this condition. A 60-year-old female presented with a lower neck massdeviating and compressing the cervical trachea.

Cervical CT revealed the tumor located in the lower neck with calcification in the right side and extended in the supero-anterior mediastinum with a deviation of the cervical trachea to the right.

Scintigrams of both ^99mTc and ²⁰¹Tl showed the tumor was composed of two parts. A transcervical approach was adopted for the complete resection of the tumor. Histological investigations revealed that the tumor was composed of cervical thymoma and thyroid papillary adenocarcinoma.
