鼻・副鼻腔に発生した無色素性悪性黒色腫の1例

Amelanotic Melanoma in the Nasal Cavity and Paranasal Sinus:A Case Report

　はじめに

　一般に悪性黒色腫は皮膚に発生する悪性腫瘍として知られているが，鼻腔をはじめとする頭頸部粘膜にも発生するといわれている。悪性黒色腫の中でもメラニンを形成せず，臨床的に色素沈着を欠くものは無色素性悪性黒色腫といわれ，臨床診断は容易ではなく予後も極めて不良とされている^1）。

　今回われわれは，病理組織学的に光顕所見のみでは横紋筋肉腫，未分化癌，嗅神経芽細胞腫，悪性リンパ腫，形質細胞腫などと鑑別困難であったが，免疫組織化学染色による病理学的検査を行い，無色素性悪性黒色腫と診断し得た症例を経験したので，若干の文献的考察を加えて報告する。

A 64-year-old woman complained of right nasal obstruction and nasal bleeding.

Intranasal biopsy of a mass suggested cancer, sarcoma or malignant melanoma.

Right total maxillectomy was performed, and HMB-45 and S-100 protein stains confirmed amelanotic malignant melanoma.

Postoperative irradiation of 50 Gy was given, and she has currently been under observation.
