一側耳下腺の良性リンパ上皮性疾患と好酸腺腫症例

A Case of Benign Lymphoepithelial Lesion and Oncocytoma in the Parotid Gland

　はじめに

　唾液腺の良性リンパ上皮性疾患（benignlymphoepithelial lesion：以下，BLLと略）は病理組織学的にはシェーグレン症候群（以下，SSと略）と同様の所見を呈し，稀に耳下腺に限局性腫瘤の形で出現することが知られている。一方，好酸腺腫（oncocytoma）は細胞質内に好酸性顆粒が充満した上皮性細胞からなる腫瘍で，頭頸部領域では耳下腺に好発するが唾液腺腫瘍全体に占める割合は1％以下と極めて低い^1）。

　われわれは一側耳下腺にBLLと好酸腺腫が出現した症例を経験した。このような症例は極めて稀と考えられるため，臨床経過に若干の文献的考察を加えて報告する。

A 68-year-old female patient was complaining of an enlargement in the right parotid gland. CT and MRI revealed two masses which were well-circumscribed in the right parotid gland. There was a high accumulation of ^99mTc in ^99mTc-pertechnetate scintigraphy. Superficial parotidectomy was perfor-med and the resected mass had two different lesions which were separated by the normal parotid glandu-lar tissue. Histologically, one lesion was benign lymphoepithelial lesion and the other one onco-cytoma. The relation between these lesions was unclear.
