下咽頭梨状窩瘻により死亡した1例

Piriform Sinus Fistula:Report of a Case

　はじめに

　下咽頭梨状窩瘻は1973年にTuckerとSkolnickが^1）前頸部腫瘍の原因のひとつとして発表し，1979年にTakaiら^2）がこれを急性甲状腺炎の原因疾患と位置づけてから年々報告例も増し，一般的に認識されるようになってきた。しかし報告症例のほとんどが若年者に限られ，高齢者における報告はほとんど見受けられない。今回われわれは若年時より左頸部の膿瘍形成，切開排膿を繰り返し，高齢となって膿汁と吐血の誤嚥により死亡した1例を経験したので報告する。

A 79-year-old man with piriform sinus fistula died of asphyxia due to bloody vomitus from gastric ulcer and pus from an abscess of the neck. He has been suffering from recurrent abscess of the left side of the anterior neck and treated by in-cision and drainage and antibiotics repeatedly since childhood. He had been complicated by un-controlled diabetes mellitus for recent lOyears. There was a huge gastric ulcer and piriform sinus fistula found at autopsy.
