甲状腺手術におけるラリンゲルマスクと神経刺激装置の応用

Thyroid Surgery using Laryngeal Mask Airway and Nerve Stimulator

　はじめに

　甲状腺の手術の際に反回神経を保存することは極めて大事であるが，挿管麻酔下では術中声帯の運動の確認が不可能であることは言うまでもない。われわれは麻酔科医の協力を得て全身麻酔にラリンゲルマスク（以後LMAと略）を用い，神経刺激装置によって反回神経を刺激し，声帯の可動性を術中モニターする方法を試みたので若干の文献的考察を加えて報告する。

It is important to preserve the recurrent laryngeal nerve during thyroid surgery, but the vocal cords can not be visualized in endotracheal anesthesia during the surgery. We observed the movement of the vocal cords by stimulation of the recurrent laryngeal nerve in the use of laryngeal mask airway in 13 patients underwent thyroid surgery. No post-operative recurrent nerve palsy was recognized. The use of the laryngeal mask airway and fiberoptic laryngoscope allowed to observe the vocal cords without removing the airway and with the use of nerve stimulator.
