悪性リンパ腫との鑑別を要したトキソプラズマ性リンパ節炎の症例

Toxoplasmosis Lymphadenitis:Report of a Case

　はじめに

　頸部リンパ節腫脹は咽喉頭炎などの炎症に付随したリンパ節炎や悪性腫瘍でしばしば認められる症候だが，時として炎症によるものなのか，腫瘍なのか，また特異的な炎症が疑われる場合その適切な治療薬の選択に迷うこともある。

　今回われわれは，初診時の症状より悪性リンパ腫が疑われたが，生検と抗体価の測定によりトキソプラズマ感染症による頸部リンパ節炎と診断された症例を経験したので報告する。

A case of cervical lymphadenitis caused by acute toxoplasmosis was reported. The patient was a 20-year-old man and visited our clinic because of multiple neck masses without pain. Palpation of the masses was strongly suggestive of malignant lymphoma. The Ga scintigraphy imaging and patho-logical examination of a lymph node were unable to diagnose lymphoma.

Serological examination by latex agglutination and the history of having cat as a pet confirmed the diagnosis of toxoplasmosis.

Sulfamethoxazol/trimethoprim were effective but medication was discontinued because of the appearance of side effects after one week. The swelling of neck lymph nodes disappeared 6 months after medication was completed.
