シスプラチン使用後難聴をきたした1症例の側頭骨病理所見—純音聴力検査とコクレアグラムとの比較

Temporal Bone Pathology associated with Cis-Platinum Ototoxicity

　はじめに

　Cisplatin （cis-diammine dichloroplatinum：CDDP）^1）は，1970年代から，頭頸部癌，泌尿生殖器系悪性腫瘍^2）に対する制癌剤として，臨床に用いられてきた^3〜6）。その毒性として腎障害，骨髄障害，内耳障害などが報告されている^7〜10）。内耳障害の病理学的研究については動物実験で広く行われており，主に外有毛細胞の障害が認められている^11,12）。ヒトでも同様に，外有毛細胞の消失をはじめとし，血管条の変性を生じ^13），難聴のタイプは高音急墜型感音難聴を呈することが多く^3,4），ある程度可逆的な変化であるとされているが，純音聴力検査と側頭骨病理を比較検討した報告はほとんどない。

　今回われわれはCDDP投与開始直前の聴力検査で正常聴力が確認され，CDDP投与後，投与量の増大とともに，聴力低下を認めた1症例の側頭骨病理所見を調べ，コクレアグラムを作成し純音聴力検査所見と比較検討したので報告する。

Pathological examinations of the temporal bone of a patient who developed hearing impairment after treatment with Cisplatin revealed that outer hair cells and stria vascularis were damaged. The outer hair cells of middle and basal turn were degenerated. Pure tone audiogram was well coincid-ed with histological cochleogram.
