頸部キャッスルマン病の1例

A Case of Castleman's Disease

　はじめに

　キャッスルマン病は1954年にCastlemanら^1）が縦隔に発生した腫瘤をhyperplasia of medias-tinal lymph nodesとして初めて報告した特異な組織像を呈するリンパ増殖性疾患である。その後Kellerら^2）が本疾患を組織学的見地からhyaline-vascular typeとplasma cell typeとに大別した。また成因については主に免疫組織化学的見地から炎症説，腫瘍説，過誤腫説など諸説があり^3,4）意見の統一をみていない。疾患の概念が未だ明確にはされていないなかで，本疾患をリンパ腫境界病変としてとらえる見方もあり悪性リンパ腫との関連が注目されている。

　本論文では頸部に発生したhyaline-vasculartypeのキャッスルマン病を報告し，本疾患と悪性リンパ腫との関連に関して文献的考察を加えた。

A case of Castleman's disease originated from the submandibular region in a 35-year-old female is reported. A tumor which was growing slowly over a period of eight years was surgically removed. His-tologically the tumor was characterized by atypical-ly rich vascularized nodular lymphoid tissue, termed as angionodular hyperplasia. Hyaline material was also noted particularly in the nodule. These charac-teristics revealed that the tumor was a hyaline-vascular type of Castleman's disease. Relationships between Castleman's disease and malignant lymphoma were discussed.
