多発性脳神経麻痺を呈したEngelmann病症例

Multiple Cranial Nerves Dysfunction in Engelmann's Disease

　はじめに

　Engelmann病は1920年にCockayneによって初めて報告された骨系統疾患であり，四肢の筋萎縮と長管骨骨幹部の骨肥厚，骨硬化を特徴とする遺伝性疾患である。1929年，Engelmann^1）によって8歳の典型例がosteopathia hyperostotica scleroticans multiplex infantilis （先天性多発性肥厚性骨症）として再度報告され，今日彼の名をとってEngelmann病と呼ばれるようになった。Engelmann病の骨病変は四肢骨に好発するが，進行して頭蓋骨にも及び，合併症として脳神経麻痺，脳圧亢進も起こすことがある。今回，経過観察中に顔面神経麻痺，顔面知覚麻痺，難聴など多発性脳神経麻痺を呈したEngelmann病を経験したので報告する。

A 23-year-old female with Engelmann's disease complained of V, VII, VIII cranial nerves dysfunction. Engelmann's disease is uncommon genetic bone dysplasia, which is the result from failure of resorp-tion of calcified cartilage and primitive bone. Conse-quently, transformations of bone are developed instead of mature adult bone. In this case, the narrowed nerve canals of the skull base seemed related to the multiple cranial nerves dysfunction.
