外鼻異物の1症例

Foreign Body in the External Nose

　はじめに

　耳鼻咽喉科領域の異物症例は気道，食道，外耳道に介在するものが中心である。部位により異物の種類は異なり，年齢による違いもある。生活様式や社会環境によっても影響を受けるため，年代によって変遷し，地域によっても差がみられる^1）。体表在性の外傷性異物迷入の頻度は低い。その多くは異物停留による遅発性症状により診断されるため，停留期間も長い。発症機転に関する病歴聴取ができない，異物刺入直後の症状がない場合など診断に苦慮する要因も加わるためと考えられる。今回われわれは1年以上経過した後，鼻屋皮下に埋没した鉄片異物を摘出した症例を経験したので文献的考察を加えて報告する。

A 42-year-old woman received a small injury at the external nose when she used a mowing machine. Six months later, the injured area had been gradu-ally swelling. Biopsy was performed and it revealed no malignancy. Because a tumorous granuloma was found under the skin, x-ray, ultrasonography and computed tomography were done and they demon-strated a foreign body in the external nose. A for-eign body with surrounding fibrous granuloma was removed under local anesthesia. It was a small iron fragment.
