　はじめに

　カフスボタン状気管カニューレ（レティナ®：以下，レティナ）は気管カニューレの長期装着による諸問題を補い，患者負担を軽減する目的で1980年に開発された^1）。それ以来多くの領域で利用されているが，レティナ装着に伴う重篤な合併症や事故は報告されていない。今回われわれはレティナを装着した2症例で，スピーチバルブ着脱の際誤ってレティナ本体を抜去し呼吸困難に陥るという突発事故に遭遇した。症例を呈示するとともに，原因とその対策について考察を加えて報告する。

A cuff botton-like cannula with a speech valve was used for a 80-year-old and a 66-year-old women with narrowness of upper respiratory tract after surgery of thyroid cancer. Both patients removed their cannula with a speech valve by accident, when they were going to take off a speech valve. One of them stifled to death, and the other survived. The accident was caused by clumsiness of the aged patients pulling the speech valve out. We recom-mend to use a newly reformed cannula for aged patients, and to educate a proper usage of the cannula and speech valve.
