Rotary Door Flap法による喉頭気管溝の閉鎖

Closure of Laryngotracheal Trough with Rotary Door Flap

　はじめに

　声門下狭窄の治療法は多種多様であるが，狭窄が高度の場合，外科的治療を要する。われわれは，高度の声門下狭窄に対し，喉頭気管溝として一定期間，頸部に開放し，拡大固定後に2期的に閉鎖する段階的手術法を用いている^1）。今回，Eliacherらが提唱したrotary door flap法^2〜5）を用いて喉頭気管溝の閉鎖を行い良好な結果を得たので手術法を中心に報告する。

A patient with subglottic stenosis was treated well by the “trough method”. Eliacher's rotary doorflap method was applied for closing a trough anter-iorly. The advantage of this flap is an axial rotation 180°, to augment the lumen of stenotic airway. This flap is available in the same surgical field without skeletal supports such as rib cartilage, auricular composite graft.
