上顎癌の頭蓋底顔面切除後の合併症の治療経験—皮弁の血行不全から始まった術後合併症を中心に

Postoperative Complications in Skull Base Surgery:Report of a Case

　はじめに

　頭蓋底外科は，最近の切除手技の進歩と形成外科的再建手術手技の向上などにより急速に進歩し，その適応は広がってきている。合併症対策も，症例の集積により進歩してきているが，ときに術後の感染，皮弁の血行不全および髄液漏などが重驚な術後合併症となり，生命的予後を大きく左右することがある。今回，前・中頭蓋底および顔面頭蓋の合併切除を行った上顎癌頭蓋底浸潤例が，術後，皮弁の血行不全により創部感染をはじめ，次々と重症合併症を併発し，不幸な転帰をとった1例を経験した。皮弁の管理の重要性と術後合併症，およびその対策を中心に報告する。

A 68-year-old man presented with maxillary carcinoma infiltrating the skull base and necessitat-ing en-bloc resection of both cranial and facial bones including the anterior and middle skull base. In this case, circulatory failure of M-C flaps result-ed in severe postoperative events such as flap ne-crosis, wound infection, CSF leakage, meningitis, brain abscess and death.

The importance of the flap management to pre-vent postoperative complications was stressed.
