A 70-year-old male developed intractable aspira-tion pneumonia after horizontal hemilaryngectomy for T2 of the supraglottic carcinoma. Since he had severe renal dysfunction and received corticoster-oids for a long period, any invasive surgery under general anesthesia was contraindicated. For this reason, the surgery of laryngotracheal separation with simple suturing of the tracheal end in the laryngeal side was performed under local anesthe-sia. The tracheal cartilages above the laryngeal end were removed for a reliable suture of the tracheal mucosa. A small fistula in the sutured tracheal end occured temporally. One month after surgery, how-ever, he comfortably ate a regular food without aspiration. The laryngotracheal separation was sug-gested to be one of the most minimally invasive sugeries to prevent aspiration.
