深い咽頭潰瘍を伴った腸管ベーチェットの1例

Intestinal Behçet with Deep Pharyngeal Ulcers: A Case Report

　はじめに

　ベーチェット病は，口腔粘膜，外陰部の潰瘍，眼症状を3主徴とする疾患として1937年Behçetにより提唱された^1）。本疾患は，これら3主症状の他にもさまざまな全身症状を伴うことが知られているが，消化管，大血管，中枢神経をおかす腸管型，血管型，神経型のベーチェットは，特殊型ベーチェットと呼ばれている^2）。今回われわれは，咽頭後壁の広範な潰瘍のため当科を受診し，中心静脈栄養開始とともに咽頭潰瘍は改善したが，その後，消化管潰瘍からの大出血のため腸管切除に至った腸管型ベーチェットの1例を経験したので報告する。

A 37-year-old man complained of severe sore throat. Deep pharyngeal ulcers were found with uveitis, ulcers of the external genitals and erythema nodosum. From these clinical findings, it wasdiagnosed as Behçet's disease (complete type). Duodenal ulcers were found by a gastric endoscopy, and then, diagnosed as an intestinal Behçet. Deep pharyngeal ulcers were healing after the therapy of intravenous hyperalimentation (IVH), colchicine and salazosulfapyridine. The duodenal ulcers were needed to have surgical treatment, because of severe bleeding from the small intestine. The patient was discharged without post operative complications.
