中耳根治術後耳に対する段階的手術—アブミ骨手術によって聴力改善を得た1症例

Stapes Surgery for Radically Operated Ear : A Case Report

　はじめに

　中耳根治手術の術後耳に対する聴力改善手術は，アブミ骨底板が固着した例では困難であると考えられている。しかし，原理的には，1）鼓膜を再建し正常粘膜で覆われた中耳腔を再建できること，2）アブミ骨の底板を除去して鼓膜の振動を内耳へ伝達すること，の2点を実現すれば聴力を改善することができる。

　中耳根治術後耳に対して，鼓膜と正常粘膜で覆われた中耳腔を再建する方法としては，著者らは全中耳再建術^1）を実施し良好な結果を得ている。また，慢性中耳炎に対するアブミ骨手術は，鼓室硬化症を対象としていくつかの報告^2〜6）があり，その有効性は確認されている。したがって，中耳根治術後耳に対しても十分な聴力改善を得られる可能性がある。

Staged surgery for radically operated ear consist-ed of three operations. The first operation was total reconstruction of the middle ear without ossicularreconstruction, the second operation tympanoplasty of the “columella-type” and the third operation was stapedectomy. A 41-year-old male received stapes surgeries in his both ears. Hearing thresholds in both ears were remarkably improved. Three years follow-up of the right ear and a year follow-up of the left year proved that stapes surgery for radically operated ear was effective and safe.
