舌小唾液腺悪性腫瘍の2症例

Two Cases of Malignant Small Salivary Gland Tumors

　はじめに

　舌の悪性腫瘍としては，扁平上皮癌が圧倒的に多く，その他の悪性腫瘍の発生は極めて少ない。発生部位の多くは舌側縁であり，舌の上縁や舌根部に発生することもあるが，その頻度は舌側縁に比べると少ない。初期より舌の疼痛が著しく，患者は比較的早期にそれを自覚することが多い。

　一方，舌に小唾液腺悪性腫瘍のできる頻度は非常に低く，その報告例もわずかである。そして，腫瘍による症状の発現が遅く，かなり大きくなってから初めて気づかれることが少なくない。1978年より1992年までの15年間に東京医科歯科大学耳鼻咽喉科において治療した舌の小唾液腺悪性腫瘍の症例は2例である。本論文においては，これらの症例について，その病理組織，治療，臨床経過について述べてみたい。

The tumor in the first case was adenoid cystic carcinoma and the other was basal cell adeno-carcinoma. The first case underwent total glos-sectomy preserving the larynx and reconstruction of the oral floor with a D-P flap. The second case underwent tumor resection with normal tissue of the base of the tongue via oral and submental approach. The first case survived for about 5 years and died of liver and lung metastasis. The second case was alive without recurrence and metastasis in March, 1993. The pathology and treatment were discussed.
