破傷風の原因となった顔面竹片異物の1症例

A Tetanus due to Bamboo Foreign Body in Face:Report of a Case

　はじめに

　近年，破傷風の発症頻度は抗生剤の進歩，DPT三種混合ワクチンや破傷風トキソイド接種の普及により減少している。しかし，発症後の死亡率は依然高く，現在でも年間30〜40人が死亡している^1）。破傷風菌は芽胞性の嫌気性グラム陽性桿菌で土壌中あるいは動物の下部消化管内に広く存在し，創部が土壌や汚水で汚染されている場合はその予防を念頭におかなければならなし。今回われわれは顔面刺創竹片異物により破傷風を発症し，破傷風治癒後も異物の発見，摘出に長期を要した1例を経験したので，顔面異物の診療上心掛けなければならない問題点などについて若干の考察を加えて報告する。

A 68-year-old female had a history of a piece of bamboo stuck into her face, and she devel-oped tetanus. She was treated with tetanus antitoxin, antibiotics, sedatives. After she was recovered from trismus, remnants of a piece of bamboo were found in the pterygopalatine fossa. The remnants were removed through incisions in the buccal mucosa and the upper lip, and her postoperative course was uneventful.
