ヘッドホーンステレオを利用したマスカー療法の試み

Treatment of Tinnitus with Band Noise Masking using A Cassette Recorder

　はじめに

　耳鳴が外界の音によって遮蔽される現象を耳鳴治療に応用し，耳鳴の遮蔽のために雑音を用いたのがマスカー療法である^1）。マスカーによる耳鳴治療では耳鳴の周波数に近い帯域雑音で完全に耳鳴が遮蔽されることが重要であり^1），従来より用いられてきたボックス補聴器型マスカー治療器は5kHz以上の周波数の耳鳴に対しては，マスキングノイズのピッチを合わせることができず，5kHz以下の耳鳴に比べ効果が劣るとされている^2）。

　今回われわれは症例ごとに耳鳴にできる限り近似し，5kHz以上の高音域でも帯域雑音を発生できる装置を開発し，それによるマスキングノイズをカセットテープに録音し，ヘッドホーンステレオ（ウォークマン；SONY）を用いてマスカー療法を試みた。

A total of 111 patients with tinnitus were trea-ted by band noise masking. We used making noise produced by a band noise generater (RION), which was adjusted to correspond to the tinnitus frequency obtained by the pitch match method.

The generater was designed so that band noise is delived over a range from 200Hz to 20kHz and the band width is changeable.

Band noise was recorded on a cassette recorder, and the patients listened to the noise for 1 hour period.

A half of the patients treated showed the disap-pearance or reduction of tinnitus.
