口腔底腫瘍と鑑別困難であった巨大な歯石の症例

A Giant Dental Calculus in the Oral Floor

　はじめに

　歯石とは歯の表面にできる硬い沈着物であり，口腔内細菌と主にCa反応性タンパク質からなる細菌間基質とから構成された歯垢，歯苔あるいは菌苔と呼ばれる軟らかい沈着物にCa，Pなどのミネラルが沈着し，石灰化したものである^1-3）。歯科領域においてはごくありふれた存在であり，一般にはその診断に苦慮することはまれと思われる。

　今回われわれは，口腔底腫瘍による下顎骨の露出と鑑別困難であった巨大な歯石を有した症例を経験し，その機序について若干の考察を加えたのでここに報告する。

A 79-year-old male presented with a painful swelling at the oral floor induced by a giant dental calculus.

The calculus, 2.5cm long and 1cm wide, situated at the midline of the oral floor. A tumor-like growth of tissue was seen around the calculus, which was completely removed. Parkinson disease and diabetes mellitus in this patient suffered for 10 years, were likely to contribute to this abnor-mal growth of calculus.
