頬部放線菌症の1症例

A Case of Actinomycosis in the Cheek

　緒言

　放線菌症は，人の常在菌である放線菌によって発症する特異的肉芽腫性炎である。放線菌は，口腔，咽頭，腸管，唾液腺，涙腺に常在している。その中でも口腔内の歯石やう歯に常在する放線菌が原因となり，放線菌症が発症する場合が多い。そのため顎口腔を含めた顔面，頸部領域が放線菌症の好発部位となっている。

　放線菌は抗生物質に感受性が高い。そのため，放線菌症は抗生剤の普及により解決されたかに思われた。しかし，近年の抗生剤の頻用傾向のためか，抗生剤の投与にもかかわらず治癒に至らない例や一時的に軽快を認めるものの再燃するといった例の報告が増加している。特に顎口腔以外の顔面，頸部領域で腫瘤を形成する例では，腫瘍に似た臨床像を呈することがあり，腫瘍との鑑別がしばしば必要となる。

A 62-year-old male complained of a swelling and redness of the right cheek. Open biopsy was performed and actinomycotic body was found in granulation tissue, which occupied the most part in the surgically excised specimen. AMPC was administered for about two months under the diagnosis of actinomycosis, and the lesion was improved.

Recently, due to abuse of antibiotic drugs, there is a high possibility that many cases of actinomycosis remained undiagnosed. We discus-sed the present situation of cervicofacial actino-mycosis.
