腺房細胞腫瘍の3症例

Acinic Cell Tumor:Report of Three Cases

　はじめに

　唾液腺悪性腫瘍のひとつに，腺房細胞腫瘍aciniccell tumorがある。これは，古くは唾液腺のaciner-like-cellからなるadenomaと分類され^1），その後悪性例も報告されたためacinic cell carcinomaと呼ばれたこともあった^2,3）が，現在では悪性の潜在能を有するlow grade malignancyとして位置付けられており，頭頸部癌取扱い規約でもcarcinomaという言葉の代りにtumorという言葉を用いている^4）。この腫瘍は，主として耳下腺に発生し，小唾液腺には稀なものとされている。

　1984〜1990年の6年間にわれわれの経験した腺房細胞腫瘍は3例あり，1例は耳下腺から発生したものであったが，他2例は上顎洞の小唾液腺類似付属腺および上咽頭の小唾液腺から発生したと考えられるものであった。これら3症例の臨床経過および病理組織学的所見を呈示するとともに，文献的考察を加えて報告する。

Acinic cell tumor of the salivary gland is a low grade malignancy having malignant potentiality.

This tumor develops chiefly in the parotid gland and is rarely found in the minor salivary glands.

We presented 3 cases of acinic cell tumor in the past 6 years. In case 1, the tumor was origi-nated from the parotid gland. In other two cases, however, the tumor have arisen from the maxil-lary sinus and epipharynx. In this paper we report the clinical course and histopathological findings of these 3 cases together with a review of the literature.
