耳鼻咽喉科手術における悪性高熱症の経験

A Case of Malignant Hyperthermia

　はじめに

　悪性高熱症（Malignant hyperthermia）は全身麻酔合併症の中では比較的稀なもののひとつである。1937年Guedel^1）により初めて症状の記載がなされて以来，多くの報告が見られるが，その原因および病態は十分明らかにされているとはいい難い。ダントロレンの臨床応用によりその死亡率は低下したといわれるが^2,3），今なお全身麻酔の重大な合併症のひとつである。われわれが文献を渉猟し得た範囲では，耳鼻咽喉科領域で過去10年間に21例の報告がある。今回われわれは，中耳根治術中に悪性高熱症が出現した症例を経験したので，文献的考察を加えて報告する。

A 63-year-old woman developed muscle rigidity of bilateral extremities, tachycardia, metabolic aci-dosis and hyperthermia (38.9℃) after administra-tion of enflurane during radical mastoidectomy under general anesthesia. She recovered immedia-tely by injection of dantrolene without any com-plications. Skinned fiber test showed a significant potention of Ca induced Ca release. The litera-ture of malignant hyperthermia was reviewed, and mechanism and treatment of this disease were discussed.
