上咽頭結核の1症例

Epipharyngeal Tuberculosis:Report of a Case

　緒言

　結核死亡率は年々減少しているが，なお欧米と比べると依然高率である。結核死亡率の改善は，抗結核剤の開発，結核対策の推進，生活水準の向上などによる。しかし，感染源として公衆衛生上問題となる感染性肺結核患者は減少の程度が少なく，自然感染率の低下による免疫をもたない若年層の未感染者の増加による集団発生の問題を新たに提起している^1）。耳鼻咽喉科領域においても依然結核は念頭において診察にあたる必要のある疾患である。われわれは右耳閉感を主訴とした46歳女性に偶然，上咽頭結核を発見したのでここに報告する。

A 46-year-old female with a complaint of full-ness in the right ear was discovered an ulcer in her epipharynx. A biopsy was taken under a nasop-haryngofiberscope. The pathological diagnosis revealed tuberculosis of the nasopharynx. With further physical examinations she was found having pulmonary tuberculosis. Sputum culture taken with a bronchofiberscope was positive for tuberculosis. The focus in the nasopharynx was regarded as a secondary lesion to pulmonary tuberculosis. Treatment with INH, REP and EB was effective.
