上咽頭結核の1症例

TUBERCULOSIS OF THE NASOPHARYNX

　I．緒言

　かつて国民病と恐れられていた結核は，学校および職場検診の徹底により，早期発見，治療がすみやかに行なわれるようになり，化学療法のめざましい発達とともに，かつてのような重篤な症例は少なくなつた。

　しかし，今なお咽喉頭結核の症例報告も少なくなく，ことに最近では病態に変貌を来たし，浸潤型が少なくなり，腫瘍型が増加している。したがつて臨床像では，悪性腫瘍との鑑別が必要となつてきた。

　A rare case of tuberculosis affecting the nasopharynx secondary to pulmonary tuberculosis is reported. A 23-year-old male medical student complained of sore throat and purulent postnasal discharge for the past two weeks. Upon examination a granular mass with whitish necrotic surface was found in the nasopharynx resembling malignant neoplasm. A biopsy was taken from this area for pathological examination. The laboratory report comfirmed the tuberculosis of the nasopharynx. X-ray examination of the chest revealed a circular cavity in the upper left lobe. The sputum examination showed positive results for smear and culture.

　The patient was placed on antituberculosis treatment with remarkable effects, both on the nasopharynx and the chest ; he is still under observation for these five years.
