甲状軟骨の変形を伴った混合型Laryngoceleの1症例

Laryngocele with Deformity of the Thyroid Cartilage: Report of a Case

はじめに

　Laryngoceleは，喉頭室前端に存在するモルガニー小嚢が空気を含み，嚢状に拡張した状態である。病因は，先天性にモルガニー小嚢の拡大に由来するもの，後天性に喉頭内圧の上昇によってモルガニー小嚢が拡大するものがある。海外での報告は比較的多いが，本邦では稀な疾患である。従って，日本語名は統一されておらず，ラリンゴケーレ，喉頭気腫，喉頭憩室，咽頭粘膜腫など種々な名称で呼ばれている。しかし，laryngoceleとそのまま述べている論文が圧倒的に多いため本論文もこれに従った。嚢胞の進展範囲により3型に分類される^1）。すなわち，甲状舌骨膜を貫いて側頸部嚢胞を形成する外側型，喉頭内内腔で嚢胞形成する内側型，およびそれらの混合型である。

　今回われわれは，反対側の甲状軟骨板の変形を伴った混合型laryngoceleを経験したので若干の文献的考察を加え，術中の嚢胞剥離時の工夫も試みたので併せて報告する。

A 63-year-old man complained of foreign body sensation in the throat and of a soft mass in the right side of the neck, which spontaneously increased and decreased in its size.

Cervical x-ray tomography and CT-scan showed an air-filled mass in the right upper neck and a deformity of the left thyroid cartilage.

The air-filled lesion was laryngocele of mixed type, and it was extirpated successfully with external approach by contact Nd-YAG laser.

He has been well after surgery.
