頸部郭清術と胸肋鎖関節肥大の関係について

Relationship between Radical Neck Dissection and Sternoclavicular Joint Hypertrophy

はじめに

　全頸部郭清術は1906年Crile^1）により提唱されて以来，頭頸部癌の頸部リンパ節転移に対する手術的治療の基本的術式として広く行われてきている。この術式において，胸鎖乳突筋と副神経を犠牲にするため術後合併症として僧帽筋を含む筋機能の運動障害によりshoulder droopが起こることがよく知られている^2）。しかし，この頸部郭清術施行後のもう一つの合併症である胸肋鎖関節肥大の出現に関する報告は本邦において皆無である。

　我々は，最近約3年間において41例の頭頸部癌患者57側に頸部郭清術を施行後，11例に胸肋鎖関節肥大を認めたので，ここに報告する。

Radical neck dissection (RND) is effective in the en-bloc removal of metastatic lymph nodes in the neck. However, sternoclavicular joint hy-pertrophy following RND appears to have escaped attention. Over the past 3 years, 32 patients with head and neck cancer had undergone classical RND in our department. Significant sternocla-vicular joint hypertrophy was noted in approxi-mately 30% of the patients. In two patients, this condition was severe enough to be differentiated both clinically and radiologically from metastasis of the head of the clavicle. Computed tomography was found to be of value in the diagnosis of the condition.
