頸部腫瘤が唯一の症状であった急性骨髄性白血病と悪性リンパ腫の同時合併症例

Simultaneous Occurrence of Non-Hodgkin's Lymphoma and Acute Myelocytic Leukemia only Manifested by Cervical Lymphadenopathy

はじめに

　悪性リンパ腫（Malignant lymphoma以下，ML）に対して化学療法や放射線療法を施行した後に急性骨髄性白血病（Acute myelocytic leukemia以下，AML）が発症してくることはそれ程稀ではないが，MLとAMLが同時に発症することは，後に述べるごとく極めて稀である。しかも，これまでの報告例では，診断が得られる前に発熱，倦怠感，全身リンパ節腫脹等の，MLやAMLの存在を示唆する症状がすでに発現しているのであるが，今回我々は，頸部リンパ節の腫脹が唯一の症状であり，そのために耳鼻咽喉科を受診し，そこでMLとAMLの合併と診断された症例を経験した。症例そのものが非常に稀であることに加え，耳鼻咽喉科だけでの診察からこうした症例が発見されたことも大変珍しいと思われたので，ここに報告する。

A 27-year-old female whose complaint was only masses in the neck was confirmed to have non-Hodgkin's lymphoma and acute myelocytic leu-kemia simultaneously by histological examination of lymph node, peripherall blood and bone marrow. There have been several previous reports of pa-tients with simultaneous occurrence of the two diseases who had no prior treatments. They all had fever, general malaise and generalized lym-phadenopathy as a complaint or physical find-ings. This case is considered to be unusual and interesting in terms of the fact that cervical lym-phadenopathy was a only manifestation.
