遊離植皮へのキチン膜（ベスキチン® W）の応用

Application of Chitin Sheet for a Free Skin Graft

はじめに

　遊離植皮を行う場合，植皮面を被覆する材料として，これまで多くのものが使用されてきた。しかし，いずれの材料を使用する場合でも，移植皮膚を確実に生着させるため，tie over sutureを必要とした。このため手術にかなりの時間を要するとともに，tie overの縫合糸による牽引のため，術後移植部の創痛を伴うことが多かった。

　今回，われわれは植皮面を被覆する材料としてキチン膜（ベスイチン^® W　ユニチカ㈱製）（ズワイガニ甲殻から抽出，精製されたキチンを繊維化して，不織布状としたものであり生体親和性，制菌性，吸水性などに優れているという特性がある）を用いることによって，このtie ovcr sutureをせずに移植皮膚を生着させることに成功したので，手術法を中心に報告する。

Chitin sheet (BeschitinR W) was applied as a covering material of the split thickness and full thickness skin graft. Four cases with a hypopharyngeal cancer in whom D-P flaps were used were reported. Free skin graft covering a skin defect caused by the elevation of D-P flap was covered by Chitin sheet without a tieover suture. In all cases, the free skin grafts were alive. By this method, it became possible that the operation time was shortened and the pain of the wound was reduced.
