 We report a case of high-risk papillary thyroid carcinoma with long-term recurrent nerve invasion. The patient is a 91-year-old man. He underwent unilateral thyroidectomy, cervical lymph node dissection, and recurrent laryngeal nerve reconstruction to avoid the risk of bilateral recurrent laryngeal nerve palsy and to improve voice function as much as possible. Nine months after surgery, improvement of vocal cord atrophy and prolongation of maximum phonation time were observed, and improvement of voice function was achieved even in patients with long-term paralysis of the recurrent nerve. In addition, no recurrence was observed 30 months postoperatively.
