 A 33-year-old man developed pain in the left temporal region and otorrhea in the left ear. He had undergone tympanoplasty for middle ear malformation 17 years previously. A bone-destroying mass with a diameter of 2.5 cm was found in the mastoid cavity and was subsequently surgically removed. The posterior fossa dura mater was exposed, but there was no cerebrospinal fluid leakage. The lesion was determined to be a cholesteatoma. Possible etiologies were:1)enlargement of congenital cholesteatoma overlooked at previous tympanoplasty or 2)post-operative formation of cholesteatoma caused by epithelial migration at the first surgery. The occurrence of such a condition is rare and difficult to predict.
