中耳奇形手術の長期経過後に乳突部に充満する側頭骨真珠腫を認めた症例

A case of temporal bone cholesteatoma occupying the mastoid cavity long after tympanoplasty for middle ear malformation

はじめに

　中耳真珠腫は術後再発をきたしやすい疾患であるが，真珠腫に対するものではない中耳手術後に真珠腫が認められる場合がある。病因としては初回手術の影響による後天的なもの^1,2）が考えやすい。

　われわれは，中耳奇形の術後17年が経過してから大きな側頭骨真珠腫を認めた症例を経験した。本症例については上記以外に先天性真珠腫が当初から合併していた可能性^3〜7）もあって興味深いため，これを報告し，病因などについての考察を加える。

　A 33-year-old man developed pain in the left temporal region and otorrhea in the left ear. He had undergone tympanoplasty for middle ear malformation 17 years previously. A bone-destroying mass with a diameter of 2.5 cm was found in the mastoid cavity and was subsequently surgically removed. The posterior fossa dura mater was exposed, but there was no cerebrospinal fluid leakage. The lesion was determined to be a cholesteatoma. Possible etiologies were：1）enlargement of congenital cholesteatoma overlooked at previous tympanoplasty or 2）post-operative formation of cholesteatoma caused by epithelial migration at the first surgery. The occurrence of such a condition is rare and difficult to predict.
