前頭洞に発症した真菌症の1例

A case of frontal sinus mycosis

はじめに

　副鼻腔真菌症は，日常診療においてしばしば遭遇する疾患であり，重篤な症状を呈する浸潤性と非浸潤性とに大別される。Bent & Kuhnの分類では，①急性浸潤性，②慢性浸潤性，③慢性非浸潤性，④真菌の抗原性が関与するアレルギー性真菌性鼻副鼻腔炎の4つに分類され，なかでも慢性非浸潤性は副鼻腔真菌症のうち最も発生頻度が高く，罹患洞としては上顎洞が最多である^1）。長谷川ら^2）は浸潤型および非浸潤型を含めた副鼻腔真菌症54例のうち83.3％が上顎洞に発生していたと報告している。これに対して，前頭洞真菌症は1.9％と非常に少数であると報告しており，前頭洞に発症する副鼻腔真菌症は稀であると考えられる。今回，われわれは左前頭洞真菌症に対して内視鏡下副鼻腔手術（ESS）を行い，良好な治療経過を得た症例を経験したため，若干の文献的考察とともに報告する。

　An 81-year-old man presented to our department with left-sided frontal headache. A computed tomography scan revealed a soft tissue shadow with a high-density area. Left frontal sinus mycosis was suspected, and endoscopic sinus surgery was performed. We observed left frontal recess constriction secondary to bone hyperplasia. Frontal sinusotomy was performed under navigation, using a curved shaver, curved drill tips, frontal sinus curette, Stanze forceps, and the Hydrodebrider System, and we detected a fungus ball in the left frontal sinus. Postoperative histopathological examination of hematoxylin-eosin stained specimens revealed conglomeration of Aspergillus, confirming the diagnosis of left frontal sinus aspergillosis. The patient has been followed-up for 11 months after surgery and has shown no recurrence.
