同側の中耳と小脳橋角部に併存した髄膜腫症例

A case of intracranial and ipsilateral middle ear meningioma

はじめに

　髄膜腫はクモ膜細胞を母地として発生し，主に頭蓋内に好発する腫瘍である。多くは良性であるが悪性のものもあり，男女比はおよそ1：2.5で女性に好発する^1）。原発性脳腫瘍のおよそ25％を占め，多くは頭蓋内に限局するが，組織型によっては頭蓋外に進展することがある^1〜5）。また，稀に頭蓋外で異所性に発生する場合もある^2,3,6）。渉猟しえた限りでは，頭蓋内に原発した髄膜腫と頭蓋外に発生した異所性髄膜腫が併存した症例は，これまでに報告がない。今回われわれは，同側の小脳橋角部と中耳に併存した髄膜腫症例を経験した。

　We report a rare case of middle ear meningioma along with intracranial meningioma on the same side. A 51-year-old woman presented with a twelve-month history of pulsatile tinnitus in her right ear. Computed tomography and magnetic resonance imaging revealed tumor lesions in her right middle ear as well as ipsilateral cerebellopontine angle. The cerebellopontine angle tumor was clinically diagnosed as a meningioma. She underwent middle ear tumor resection, and the histopathological examination revealed grade Ⅰ meningothelial meningioma. Her post-treatment course remained uneventful over 12 months after the surgery. We concluded that the ectopic middle ear meningioma and the primary intracranial meningioma were coexisting in this case.
