2型糖尿病とSjögren症候群を背景とした副耳下腺放線菌症の1例

A case of actinomycosis of the accessory parotid gland in a patient with type 2 diabetes mellitus and Sjögren's syndrome

はじめに

　放線菌症はActinomyces属の細菌によって引き起こされる感染症で，頭頸部領域に好発する。唾液腺発生は稀であり^1,2），副耳下腺においてはいまだ報告がない。今回，われわれは副耳下腺に発生した放線菌症を経験した。放線菌症の発症リスク因子として，口腔内不潔や易感染状態であることが挙げられるが，本症例もコントロール不良の2型糖尿病とSjögren症候群を背景に発症したと考えられた。放線菌症の治療はペニシリンなどの抗菌薬投与が基本であるが，併存疾患のコントロールを行うことで良好な転機を得られたため報告する。

　Actinomycosis is a rare chronic disease that occurs most commonly in the cervicofacial region. We reported the first case of actinomycosis of the accessory parotid gland in a patient with poorly controlled type 2 diabetes mellitus and Sjögren's Syndrome. The common treatment for this condition is antibiotics, but the duration of treatment has not been established. Even though our patient only received antibiotics for a relatively short period of time （approximately three weeks）, a favorable response was achieved through the management of the underlying condition.
