遊離空腸再建術後の腸間膜リンパ節に転移をきたした1症例

Recurrent metastasis in the mesenteric lymph node of a free jejunal graft

はじめに

　進行下咽頭がん，頸部食道がんに対する手術として，下咽頭喉頭頸部食道摘出術および遊離空腸による再建は現在，標準治療として確立されている。経過観察中の腸間膜リンパ節の腫大は少なからず認めるが，腸間膜リンパ節転移をきたした症例は稀であり，その対応法は確立していない。今回，われわれは異時的に多発した頭頸部がんに対する根治切除術および再建術術後に腸間膜リンパ節転移をきたし，リンパ節摘出を行った症例を経験したので，文献的考察を加えて報告する。

　We presented a 67-year-old man who had recurrent metastasis in the mesenteric lymph nodes of a transplanted jejunum after several surgeries for head and neck cancer. Since metastatic lymph node was on the pedicle of a free jejunal graft, we carried out resection of the mesenteric lymph node without resection of nutrient vessels. Several months after this surgery, CT revealed metastasis in the mesenteric lymph nodes and oropharyngeal space.
