外側半規管vHITの温度刺激検査とcVEMPとの相関についての検討

Correlation between the video head impulse test（vHIT）of the horizontal canal, the caloric test, and the cervical vestibular-evoked myogenic potential（cVEMP）

はじめに

　末梢前庭器（半規管と耳石器）が重要な役割を果たす前庭動眼反射（vestibuloocular reflex：VOR）は頭部の動きを代償する眼球運動であり，特に半規管は頭部の急速で高周波数の運動に対しても非常に精密かつ迅速に応答する反射である^1,2）。末梢前庭機能検査においては，現在に至るまで回転刺激検査や温度刺激検査が主役を担ってきたが，いずれもやや侵襲的であり，検査に時間を要することが問題であった。一方，1988年にHalmagyiとCurthoys^3）により報告されたhead impulse test（HIT）は，一側末梢前庭機能障害によりVORゲインに左右差が生じることを利用して，簡便に半規管機能の左右差を検出できる検査である。最初に報告されたHITは定性的な検査であったが，その後，高速CCDカメラと加速度計を備えたvideo head impulse test（vHIT）^4）が出現し，半規管機能を定量的に評価できるようになった。現在は外側半規管に加えて前半規管と後半規管の機能も個別に検査できるが，手技に習熟が必要なこと，日本人では眼裂が狭小な場合も多いことなどから，前半規管および後半規管の検査には困難を伴うことも多い。すなわち，最も手技的に容易で困難が少ないvHITである外側半規管のvHITのみ行って，上前庭神経系の機能に加えて下前庭神経系の機能を部分的にでも推測できれば，vHITはより有用な検査として普及が進むと考えられる。

　今回，外側半規管のvHITを用いて外側半規管から上前庭神経系の機能と下前庭神経系の機能をどの程度推測できるか検討するため，温度刺激検査，胸鎖乳突筋由来の前庭誘発筋電位検査（cVEMP）を行い，外側半規管のvHITとの相関について評価したので報告する。

　We investigated the correlation between the vHIT of the horizontal canal, the caloric test, and the cVEMP, in 45 patients diagnosed with vestibular neuritis or unilateral vestibular neuroma. There was a strong correlation between the results of the vestibulo-ocular reflex（VOR）gain asymmetry in the horizontal canal vHIT and the percentage of canal paresis in the caloric test. However, no correlation between the results of the VOR gain asymmetry on the horizontal canal vHIT and the cVEMP response was observed. Using results of the horizontal vHIT, we can estimate the functional roles of the horizontal semicircular canal and the superior vestibular nerve；however, this test cannot be used to investigate the inferior nerve function.
