 Free jejunum transfer (FJT) is a well-established method for hypopharyngeal reconstruction and is known to be associated with minimal postoperative complications. Although postoperative strictures at the esophageal anastomosis are uncommon, we encountered a case of persistent esophageal stricture following FJT. The patient was a 69-year-old man who had undergone a total laryngopharyngoesophagectomy and FJT, followed by postoperative chemoradiation therapy. He developed progressive dysphagia beginning at 10 months postoperatively and was subsequently referred to our department for surgical intervention. The initial treatment, a partial resection of the constricted segment of the transferred jejunum, was unsuccessful. Ultimately, replacement of the entire jejunal graft was performed one year after the initial treatment, enabling the patient to resume oral feeding.
