詳細な聴覚平衡覚検査を行った脊髄小脳変性症症例

Neurotological examination for a diagnosis of spinocerebellar degeneration—case report

はじめに

　脊髄小脳変性症は進行性の変性疾患で，「運動失調を主症候とし，小脳，脊髄に病変の首座をもつ原因不明の変性疾患の総称」である。主症状は不安定歩行，四肢機能障害，眼球運動障害，構音障害であり，めまい・ふらつきを主訴として耳鼻咽喉科を受診することもある疾患である。

　今回われわれは，神経耳科学検査が小脳機能評価に有用であった脊髄小脳変性症例を経験した。本症例の診断過程や臨床的特徴について若干の文献的考察を含めて報告する。

　We presented the patient with spinocerebellar degeneration（SCD）who had an eight year history of tremor and gait disturbance. Neurotological examination was quite useful for a diagnosis of SCD in such patient. Using electronystagmography（ENG）, we observed spontaneous down beat nystagmus and saccadic in the eye tracking test（ETT）. These were positive indicators that the abnormality was associated with the central cerebellar function and not with the peripheral vestibular function. Additionally, characteristic cerebellum atrophy was found using magnetic resonance imaging（MRI）.
