放射線治療後に紡錘形細胞癌として再発した声門扁平上皮癌症例

A case of spindle cell carcinoma transformed recurrently from glottic squamous cell carcinoma followed by radiotherapy

はじめに

　紡錘形細胞癌（spindle cell carcinoma：SpCC）は，上皮内または浸潤性扁平上皮癌（squamous cell carcinoma：SCC）と，上皮性であるが間葉系腫瘍の様相を呈する紡錘形腫瘍細胞の2相性細胞で構成された特異な腫瘍である^1）。

　喉頭におけるde novo発生は喉頭悪性腫瘍の1.7〜2.7％と報告されているが^1,3），放射線治療したSCCが再発したあとSpCCと診断された報告は稀である^1）。

　今回われわれは，左声門SCCを一次治療したあと，気管，声門下と2度の再発・転移を繰り返し，最終的にSpCCの病理組織学的診断となったが制御された1症例について，若干の考察を加えここに報告する。

　Spindle cell carcinoma recurrently transformed from glottic squamous cell carcinoma is rare.

　A 72-year-old man presented with hoarsness was diagnosed as glottic squamous cell carcinoma and treated by induction chemotherapy and definitive radiation. Two years later the carcinoma recurred in the trachea, which disappeared after the same therapeutic manner. However because of carcinomatous web-formation in subglottic portion six-months later, total laryngectomy was undertaken. Histopathological examinations revealed transformation of squamous cell carcinoma into spindle cell carcinoma. After 8 years point he died of pneumonia without any recurrence.
