当科における声門上癌症例の検討

はじめに

　喉頭癌は頭頸部領域で発生頻度の高い悪性腫瘍で，声門上癌は頸部リンパ節転移をきたしやすく，遠隔転移が多い。治療法は放射線療法，化学放射線療法，喉頭全摘術，喉頭温存手術と多岐にわたり，T2症例，T3症例での喉頭温存率，治療の選択などに関しては施設間差が存在する。今回われわれの施設において，今後の方針を再検討するために過去13年間に当科で一次治療を行った声門上癌症例について検討を行ったので，若干の文献的考察を加えて報告する。

　We reviewed 81 cases of supraglottic carcinoma. The five-year cause-specific survival rates determined using the Kaplan-Meier method were 100％ for stage Ⅰ, 79％ for stage Ⅱ, 81％ for stage Ⅲ, 59％ for stage Ⅳ. Total laryngectomy was performed in most cases in T3 and T4. Larynx preservation rate was low. Cases of T3, T4 with or without invasion of the paraglottc space（PGS）and preepiglottic space（PES below preepiglottic space）were 26 cases.

　Distant metastasis was found in 5 of 26. Patients with distant metastasis showed PGS invasion in all cases.
