巨大な先天性真珠腫と中頭蓋窩手術

A Large Primary Cholesteatoma in the Middle Cranial Fossa

はじめに

　中耳に発生した真珠腫が，内耳破壊^1），頭蓋底侵入^2），時に頭蓋内合併症^3）を併発することがよく知られている。一般にそのようなケースは中耳炎に続発することが多く，後天性真珠腫である。最近われわれは，内耳障害を呈した患者で，錐体骨を破壊し，中頭蓋窩に進展した巨大な先天性と考えられる真珠腫を脳外科医との協力で経中頭蓋窩法で摘出した。ここにその症例を報告し手術法について紹介する。

A 68-year-old male who complained of a hearing loss and vertigo showed a large cholesteatoma extended into the middle cranial fossa was reported. Roentgenological examinations including CT and MRI revealed a destructive lesion in the petrous bone and the middle cranial base. A large cholesteatoma involving the tympanic tegmen area was removed by utilizing the middle fossa approach.
