下咽頭の食物異物

Three Cases of Hypopharyngeal Foreign Body

I．はじめに

　気管や食道の異物は医療技術の進歩の著しい今日でも時には生命にかかわることがあり，数多く報告されている。これに対して，気管と食道の分岐部直上にあたる下咽頭の異物には魚骨のような尖鋭な異物が多く，梨状窩，舌根部，喉頭蓋谷などにしばしば認められるが，臨床的には危険な事態を惹き起こすことは稀であるためか，下咽頭異物は気管や食道の異物に比べると臨床では決して少なくない^1,2）にもかかわらずその報告例は少ない^3〜11）。

　最近われわれは下咽頭異物としては比較的稀と思われた下咽頭の食片異物を3例経験したので，若干の考察を加えて報告する。

Three cases of hypopharyngcal obstruction withforeign bodies in a 68-years-old man, a 64-year-old man and a 54-year-old man. They were suffering from several handicap such as dementia, dysphagia, dysarthria and aphasia due to cerebral vascular diseases. Their foreign bodies were chikuwa (a kind of fish sausage), lotus root and kon-nyaku(devil's tongue). The third patient died of airway obstruction due to kon-nyaku. In patients with denture, dementia, dysarthria and dysphagia, careful adjustment in food and way of eating was neces-sary in order to prevent hypopharyngeal foreign body such as foods.
