　はじめに

　1957年にSeidenbergら^1）により，頸部食道癌の再発のため下咽頭，喉頭，頸部食道摘出が行われた症例に対し初めて遊離空腸移植術が用いられた。その後顕微鏡下に行う血管吻合手技の導入や普及に伴い，現在本術式は下咽頭頸部食道癌切除術後の再建術式となっている^2）。

　当科においても，約2年前より下咽頭癌症例に対して遊離空腸による再建を行っており，今回はこれらの術後合併症および嚥下機能と摂食状況を中心に検討した。

We studied the postoperative complications and alimentation of 26 patients with hypopharyngeal carcinoma whose pharynx was reconstructed usinga free jejunum flap. Complications were encounter-ed in 20 occasions in 13 patients. Mean period until self alimentation was 19.3±8.8 days (n=20);13 patients ate normally and 7 patients partook of gruel;16 patients ate within 40 minutes per each meal and 4 patients took more than 60 minutes per each meal. Our study showed that the higher the excision site in the pharynx and if the reconstruc-tion of the pharynx involves a reversed jejunum, then the vermiculation may deteriorate during alimentation.
