顎下腺腫瘍の臨床ならびに病理学的検討

Clinical and pathological examination of submandibular gland tumor

はじめに

　顎下腺腫瘍は，唾液腺腫瘍のなかで耳下腺腫瘍に次いで発生頻度が高い。その組織型は耳下腺同様多岐にわたり，悪性腫瘍については手術法・郭清範囲・予後について詳細に述べられた報告は少なく，治療法が確立していない。今回，われわれは過去17年間に当科で診断・加療を行った，顎下腺腫瘍について臨床的・病理学的に検討を行ったので報告する。

　A clinical study was performed on 51 patients with submandibular gland tumors initially treated in our hospital from 1996 to 2012. Benign tumors were 36 cases, the malignant tumor 15 cases. All 36 benign tumors were pleomorphic adenoma. Proper diagnosis rate of the fine needle aspiration biopsy was 89％. A curative treatment was provided for 12 malignant tumors. Among all 12 patients, there were 5 cases showing recurrence. Recurrent site was cervical parts in 2, and, metastases to lung in three. The disease-specific accumulation 5-year survival rate was 50％.
