地方小規模病院におけるめまい疾患の統計―高齢者を中心として

Statistical analysis of elderly patients with vertigo or dizziness

Ⅰ．はじめに

　めまいを訴えて耳鼻咽喉科外来を受診する患者は多く，その原因疾患は多岐にわたる。また，これまでの臨床統計からは，対象年齢や，対象施設が大学病院か市中病院，診療所かによっても，原因疾患の頻度に違いが存在することが知られている^1～10）。

　福島県立南会津病院（当院）は，福島県の南西に位置する，病床数100床の小規模病院である。当院の診療圏である南会津郡は，おおよそ神奈川県に匹敵する広い面積であるが，人口は約2万9千人（2011年11月）と少ない。また，南会津郡は人口における65歳以上の高齢者が占める割合（高齢化率）が36％（2008年）と高く，当院も高齢者の受診が多いのが特徴である。

　今回，われわれは地方小規模病院である当院における，めまい疾患の臨床統計を検討したので報告する。

　We reported a statistical analysis of 240（male 91, female 149, mean 64 years-old）patients who visited Fukushima Prefectural Minami-aizu Hospital complaining of vertigo or dizziness from July 2007 to June 2011. Most frequent diagnosis was vertebro-basilar insufficiency（VBI）（22.9％）, followed by benign paroxysmal positional vertigo（BPPV）（20.9％, including suspicous cases）, cervical vertigo（13.3％）, Meniere's disease（7.8％, including suspicious cases）, and sudden deafness with vertigo（6.3％）. Our hospital is a small rural hospital, and the frequency of VBI tends to be higher than that in university hospitals.
