小脳梗塞を伴った片頭痛関連めまいの1症例

A case of migraine associated with vertigo after cerebellar infarction

はじめに

　片頭痛関連めまい（migraine associated vertigo）は，近年その疾患概念が認知されてきているものの，いまだに耳鼻咽喉科医には十分に知られていない疾患である。また，疾患名も統一されておらず，片頭痛性めまい^1），片頭痛関連めまい^2,5,7），片頭痛関連性めまい^3），片頭痛関連のめまいなど^4），多くの名称が用いられているのが現状である。近年は「片頭痛関連めまい」の疾患名が用いられることが多いため，本稿でも，これに倣うことにした。また，片頭痛の分類の中に，脳底型片頭痛と呼ばれるものがあり，これは脳血管が収縮的に働くことにより前兆や，構音障害，回転性めまい，耳鳴といった随伴症状が引き起こされると考えられているが，耳鼻咽喉科医にとっては，さらに稀な疾患と思われる。

　今回，当院で小脳梗塞を伴った片頭痛関連めまいの1症例を経験した。経過からは，脳底型片頭痛が疑われた症例であるが，片頭痛関連めまいや脳底型片頭痛に脳梗塞を合併することは稀^8～12）であるため，本症例を報告する。

　We reported a rare case of migraine associated with vertigo accompanying with cerebellar infarction. A 22-year-old man visited our hospital was complaining of right-sided throbbing headache, vertigo and vomiting. The headache was not accompanied by tinnitus, hearing loss, photophobia, phonophobia and other neurological symptoms. Nystagmus was not observed. Pure-tone audiometry was normal. MRI showed old right cerebellar infarction. Diagnosis of migraine associated with vertigo was established, and basilar migraine was suspected although the symptoms did not fulfill the diagnostic criteria.
